- The $100 discount offer is valid for crowns, bridges, veneers, inlays, onlays, and dental implants only where the total treatment cost is $750 or more (inclusive of gst).
- You must be 18 years old or older at the time of receiving the dental treatments and accepting these T&Cs.
- The paper voucher must be presented at the time of payment and cannot be redeemed for cash. The Offer is limited to one voucher per patient. Vouchers are non-transferable and cannot be used in conjunction with any other offer.
- The Offer is valid until 31/12/2024, subject to appointment availability.
A consultation may be required before any treatment is performed. All treatments are provided based on the dentist’s clinical assessment of whether the treatment included in the Offer is suitable for you. If we consider it is not, i.e. you may need alternative/additional treatments, we will provide details of those treatments to you, including details of our standard rates. Any additional or unforeseen treatments required following the consultation are not covered by this offer.
- You may be required to fill out a medical history form when you attend the NDC practice (and prior to your consultation) to assist our dentist’s evaluation. There is no obligation for you to proceed with the dental treatment following the consultation.
- While we undertake best endeavours to support you to achieve your desired results, individual responses and individual results vary. Outcomes experienced by one person do not necessarily reflect the outcomes that other people may experience, as results may vary due to many factors, including the individual’s genetics, diet and exercise.
- All dental procedures carry risks. Please consult your dentist about the risks and benefits specific to your treatment before proceeding.
- Some dental treatments may involve the use of products that are not available for purchase by the general public and may only be offered following consultation with one of our dentists.
- We will endeavour to provide you with reasonable notice prior to varying material terms of the Offer. You can choose to withdraw from participating in the Offer at any time.
If you do not have private health insurance cover, you will receive $100 off the total cost of your treatment provided the total treatment cost is $750 or more.
If you have a private health fund policy, it is your responsibility to check your health fund policy to determine which portion of your fee is covered. You will receive $100 off any gap payment you may be liable for provided the overall treatment cost is $750 or more. Should the gap payment be less than $100 (before the application of the Offer), you will not need to pay for any out of pocket costs but cannot otherwise redeem any excess value for cash or for any other offer.
While our practices accept all major health funds, not all health funds offer the same level of benefits and your level of cover will affect your “out-of-pocket” costs. In addition, many of our practices are classified as preferred providers for selected health funds. This allows our patients to gain access to additional exclusive benefits, depending on their health fund and level of health cover.
- We will comply with all our obligations under the Australian Consumer Law in relation to the services provided to you, including complying with all mandatory requirements regarding consumer guarantees.
- Subject to the consumer rights and warranties described above, and to the extent permitted by applicable law, we, our employees, contractors and agents will not be liable to you or any third party whether in contract, common law, tort, equity, under statute, in restitution or otherwise, in respect of the services for:
- any indirect, consequential, exemplary, punitive or special losses including loss of profits, loss of opportunity and loss of goodwill; or
- any injury, damage or loss to any person resulting from your actions in connection with our services.
- These T&Cs are governed by the laws of New South Wales, Australia.
- These T&Cs incorporate our Privacy Policy (https://www.gracedentalgroup.com.au/privacy), the terms of the practice policy and payment terms on our website (https://www.gracedentalgroup.com.au/terms), any consultation treatment letter, and quotations provided by us and accepted by you.
You agree to be bound by these T&Cs. If you do not agree to these T&Cs, you must let us know immediately and we will not provide any further services to you.
